To explain idiots like arch you have to use following logic.

Bomb explodes with the mafia.

Then the doctor visits the bomb in hospital and revives/saves his life.

Doctor is not like your security guard. Doctor saves you after you have been attacked fatally.

for the record, the reverse is also true. if mafia kills bomb but first-voting mafia is saved, then the mafia is saved (but with doc outting it, becomes confirmed mafia).
