Why is this a thing?

Think about the game in isolation. It's better across games without a doubt to save the clear, because the point is that mafia shouldn't be allowed to do it (And it SHOULD be expected that doctor would be on the clear, every game). But gamethrowing is something done in an individual game. In much the same way saying "I am town" or "I am mafia" with 100% honesty will be more advantageous to you in the longrun, it doesn't change the fact that in individual games it's still throwing.

With this idea of the game as a single instance in mind, think about the state of the game on meteor night. If the doctor successfully saves, he 100% loses-it doesn't matter who he was on. If he doesn't save anyone, including the clear, he does not 100% lose. It doesn't matter too much that mafia isn't SUPPOSED to go for the clear, it matters that he took an action which will 100% lead to a loss if successful.

This is the problem with "sporting" being the guidance that I mentioned earlier.
